Medical transcription: Outsourcing helps increase efficiency and improve services

Medical transcription has played an important role in the process of healthcare by helping document the patient-healthcare professional encounter. Medical transcription is also the first step in the process of reimbursement as it provides the basis for coding and billing.

In a time where healthcare facilities are struggling with shrinking incomes and galloping expenses, outsourcing medical transcription is an easy fix to curb costs of transcription.  Apart from curbing costs outsourcing medical transcription can help the healthcare facility increase efficiency and improve services.

How does outsourcing medical transcription help increase efficiency?

Outsourcing medical transcription helps increase efficiency of the healthcare facilities in the following areas:

·        Improving report accessibility: Outsourcing medical transcription reduces the burden of the healthcare facility support staff, by creating an orderly system for collection of audio files, uploading dictation, distribution of finished transcripts and providing additional features like electronic signature, remote printing & faxing etc. Outsourcing medical transcription also helps in tracing files through an efficient system, enabling the support staff to track the status of work in progress, at any given time. Access to patient records is also improved by use of an efficient archiving systems

·        Saving on resources:  Outsourcing medical transcription frees up valuable resources of the healthcare facility like real estate, capital expenditure on transcription equipment, office space, information technology staff, management resources and working capital.  These resources can then be utilized towards the core business of the healthcare facility, thus improving efficiency

·        One stop solution for medical transcription needs: Outsourcing medical transcription ensures that the service provider is a one stop for all the problems associated with medical transcription. This would include variation in transcription volumes, holiday & weekend transcription, addition of new specialties, inclusion of new report formats/templates, integration with EMR/EHR, and HIPAA & HITECH concerns for security of information during the medical transcription process

·        Accelerating funds flow:  Outsourcing medical transcription ensures that medical records are created on time. This ensures that the first step in the receivables cycle takes place on time. This in turn helps accelerate the reimbursement process. Quickening the receivables cycle helps the funds flow and enables the healthcare facility to perform efficiently

How does outsourcing medical transcription help improve services?

·        Improved quality of reports: Outsourcing medical transcription has added benefits like medical transcriptionists trained in specialties, multiple quality checks and the past experience & proficiency of the service provider in serving diverse healthcare facilities helps improve the quality of reports. The improved quality of the information in the patient records helps the healthcare facility improve quality of services by helping healthcare professionals make informed decisions

·        Improved turnaround time: Outsourcing medical transcription ensures that medical records are created on time. Having the right information on hand in a timely basis helps healthcare professionals improve quality of services to patients

It can be concluded that outsourcing medical transcription provides a variety of advantages to the healthcare facility. To benefit from the advantages of outsource medical transcription, it is important to analyze the specific needs of the healthcare facility and source a medical transcription company who can provide services accordingly.

The capabilities of the medical transcription service provider needs to be evaluated on the following criteria:

  • Accuracy: This means the percentage of accuracy of the completed transcripts vis-à-vis what was dictated. A minimum of 99% and above would be acceptable.

  • Turnaround time: Turnaround time refers to the time taken to revert the transcribed report to the healthcare facility. A Turnaround time of not more than 24 hours for normal reports and 4-12 hours for STAT reports would be considered ideal.

  • Data security: The service provider being outsourced to should be HIPAA and HITECH compliant. The medical transcription service provider has to have adequate security measures to ensure that the confidential data transmitted back and forth is safe and secure.

  • Pricing: Reduced costs are one of the main benefits that a healthcare facility hopes to have, from the outsourcing option. Pricing should be fair and reasonable for the services rendered. Pricing method should also be transparent and simple so that both parties can verify the same at any stage of the process

Medical transcription outsourcing & Data security

Is outsourcing of patient records really unsafe?

There have been serious doubts about data security while outsourcing medical transcription. The case of Lubna Baloch from Pakistan who threatened to expose confidential data over the Internet for non-payment for services rendered has been used as an argument against outsourcing.

It is understandable that situations like this are worrisome and very traumatic for all parties concerned. But can this be cited as a case against outsourcing? To answer that question one needs to look at the case in question in great detail.

Let us examine the chain of events that led to the event in question:

UCSF medical Center outsourced it medical transcription work to
¯
Sausalito’s Transcription Stat who outsourced it to
¯
15 contractors throughout the country
¯
One of the 15 contractors named Sonya Newburn further subcontracted the work to
¯
Tom Spires who subcontracted to
¯
Lubna Baloch

It can clearly be seen that the healthcare facility originally outsourcing its’ medical transcription needs had no idea as to who is actually doing their work. Even the medical transcription service provider who was under contract with them had no control over the end transcriber.

The lesson to be learnt from this situation is that while outsourcing the medical transcription needs of a healthcare facility, care should be taken to ensure to choose a medical transcription service provider that does not use sub contractors for executing the work. Due diligence on the working methods of the medical transcription service provider should be carried out; care should be taken at the time of drafting the contract to ensure that the “no sub contractor clause” is included.

TransDyne is one company, which believes it is their privilege to be serving their customers’ needs in the best way possible. So any work outsourced to TransDyne, is executed by their own team of trained Medical Transcriptionists, than by the use of subcontractors or home transcriptionists.

Apart from ensuring that the data never leaves the premises physically, TransDyne ensures security during the medical transcription process by incorporating security measures like 128-bit data encryption, multi-tiered application architecture, design level security, safeguards, firewall protected networks, sterilized e-mail servers, denial of access procedure and multi modal alerts.

Further security is ensured by manned security at entry and exit points, surveillance cameras, disallowance of any removable storage devices and limited access of systems and information on a need to know basis. The people working on the transcription have also been educated on the importance of being HIPAA and HITECH compliant.  Undertakings have been taken from the personnel to further ensure confidentiality.

Choosing a professional and committed medical transcription service provider like TransDyne would ensure that the healthcare facility would enjoy not only the cost saving benefits of outsourcing and the knowledge that patient data confidentiality is maintained but also the other benefits of outsourcing like:

1.      Reducing capital investment with regards to medical transcription
2.      Managing fluctuating transcription report volumes and staffing issues for holidays and weekends.
3.      Reducing management expenses towards medical transcription
4.      Reduce human resources expenses connected to medical transcription
5.      Reduce direct labor cost
6.      Reduce It expenses
7.      Reduce real estate cost pertaining to medical transcription
8.      Improve information access
9.      Improve turnaround time
10. Shorten the receivables cycle
11. Concentrate on core business
12. Take advantage of a single source solution

TransDyne offers quality medical transcription at reasonable prices, executed by experienced and qualified medical transcriptionists with a very quick turnaround time executed through secure HIPAA and HITECH compliant channels, with very high levels of accuracy and all this with technology that is advanced but easy to use!

Medical transcription: A necessity or luxury?

To assess whether medical transcription is a necessity or luxury one needs to look at the history of medical transcription and the role it plays in the provision of quality care and the profitability of a healthcare facility.

·        Evolution of medical transcription: Medical transcription is process of creating records of patient-healthcare professional encounter. Creation of records of the patient encounter has been around for a long time. Over time medical transcription has evolved from its’ early stages due to the advance of technology and the Internet. The various stages of evolution that medical transcription has gone through are as follows:

Ø     Hand written notes
Ø     Dictation-shorthand-typing on the manual typewriter
Ø     Dictation- recording on tapes- transcribing on manual/electronic typewriter.
Ø     Uploading dictation on the computer system, downloading, transcribing and uploading finished files and then transmission back to the healthcare professional through the use of Internet.

·        Role of medical transcription: Medical transcription has always played a behind the scene role in the form of an essential support to the process of healthcare.  Medical transcription plays an important role in the following:

Ø     Recording the patient-healthcare professional encounter
Ø     Referrals/ tests
Ø     Statutory requirement
Ø     Insurance re-imbursement
Ø     Evidence in case of litigation
Ø     As part of patient medical history.

One can see that medical transcription plays an important role in the core business of a healthcare facility and its’ profitability. It would be considered a necessary service rather than a luxury.

How can this necessary function of medical transcription be made economical and efficient?

Medical transcription can be made economical and efficient by outsourcing to a professionally managed medical transcription company, which needs to be evaluated on the following:

  • Quality/accuracy: Quality of the transcribed document refers to the percentage of accuracy in the transcribed document viz. a viz. the audio files.  Accuracy level assured by the medical transcription service provider should be as close to 100% as possible

·        Turnaround time: Turnaround time refers to the period between uploading the audio file to the time when the finished record is made available to the healthcare professional. A turnaround time of 24 hours with quicker turnaround time options for STAT reports would be considered reasonable.

·        Data security: Security of confidential patient data is an important consideration. This becomes even more critical when the data is transmitted back and forth. Any prospective medical transcription service provider needs to be critically evaluated in terms of the measures taken by them to ensure security of data.

·        Price: Cost savings per line of transcription is perhaps one of most important considerations for outsourcing medical transcription needs. The pricing provided by the medical transcription service provider needs to be reasonable and the terms of pricing should be easy to understand.

·        Volume flexibility: It is not possible to predict the volume of patient inflow. Therefore it is not possible to predict the volume of medical transcription work required. A medical transcription service provider needs to be evaluated on the basis of their ability to cope up with varying volumes and factors like weekend and holiday transcription requirements.

·        Right use of technology: Technology plays an important part in the medical transcription process. It can make the process simpler, quicker and safer by providing easy methods of capturing dictation, uploading files, downloading completed records and even for archiving of patient records. Tracing the information required can be made easier through the use of technology. The medical transcription service provider should be evaluated on the use of technology in the medical transcription process.

·        Billing methods: The billing method used by the medical transcription service provider can have a direct impact on the profitability of the healthcare facility. The billing method needs to explained at the beginning of the process. The billing method should be evaluated to ensure that it is, verifiable, definable, measurable, consistent and fair & honest.

Medical transcription: Flexible services

It is a constantly changing world, needs are also constantly changing. It is the onus of the service providers to be flexible to meet the changing requirements of their target customers. Medical transcription is a service that has been allied with the healthcare sector by converting the audio records of the patient-healthcare professional encounter into text format. To fulfill the varying needs of the healthcare sector it is important that medical transcription services be flexible.

What are the factors that could affect medical transcription?

  • Varying influx of patients: The influx of patients is not something that can be predicted. It is important for medical transcription service providers to be prepared for varying volumes of transcription depending on the inflow of patients

  • Turnaround time requirements: Different specialties have different turnaround time requirements. Sometimes the healthcare professional may require the report to be transcribed instantly and sometimes a time lag does not affect the process of healthcare. A medical transcription service provider has to be in a position to meet the varying turnaround time requirements

  • New specialties being added to existing healthcare facilities: A medical transcription service provider may be used to transcribing only for certain specialties. When new specialties are added the medical transcription service provider has to develop competency in transcribing for the new specialty as well as keeping up with the current demands.

  • Expansion plans: Healthcare facilities may have plans to expand geographically as well as in other ways. This could mean that the medical transcription service provider needs to adjust to various new factors like new healthcare professionals, new specialties, new formats, different accents, different time zones, different turnaround time expectations and even different systems

  • Different formats required by different specialists:  Medical transcription service providers have to keep up with the demands of different formats of reports required by different specialists.

  • New developments in the field of healthcare: Healthcare is a field that has constant developments. This could be in the form of discovering new ailments, new therapies and invention of new drugs and procedures. A competent medical transcription service provider has to constantly update its’ team of medical transcriptionists to enable them to provide expert medical transcription services.

  • Adoption of EMR: Healthcare facilities may decide to implement an EMR/EHR system. This would require a medical transcription service provider to enable entry of information into the EMR/EHR via HL7 interface.

  • Adoption of voice recognition software: Sometimes the medical transcription service provider has to adopt the role of proofreader/editor when healthcare facilities have adopted voice recognition software for their medical transcription needs.

Being associated with healthcare, medical transcription services have to be flexible to meet the varying needs.

TransDyne, a leader in medical transcription believes in being flexible to meet the varying needs of healthcare facilities.

Medical transcription: A case study for outsourcing

Dr.Williams, 37, was based in New Jersey in an upscale neighborhood. He was a general physician and had a well-established practice. He had a small clinic right next to his home, a staff of two, a nurse and a receptionist cum secretary. He also had a part time accountant who took care of filing reimbursement forms, bank work and tax filing.

The nurse, Sarah would help him with initial information collection like patient name, social security number, details of the healthcare plan, demographics and assisted him while examining patients, administering injections, checking blood pressure etc. The receptionist/secretary Jane was in charge of scheduling patients, handling patients when they came in and transcribing the doctors notes once he had finished dictating his notes.

Dr.Williams found that he was constantly juggling things, and he could never find the information he wanted on time. He could not actually find fault with Jane as she seemed to be overloaded with work. She constantly seemed to be juggling phone calls, people in the reception, typing out files and maintaining the filing system. He was also informed by Robert the accountant that his fund flow was low, as filing for claims seemed to take an inordinate time, owing to the fact that he could never find information on time.

Dr.Williams would observe whenever there was an additional influx of patients he would receive reports as late as seven days including for those patients for whom he had ordered further tests. This would hinder him from making the appropriate treatment plans. He would find himself relying on scribbled notes, his own memory and the inputs provided by his nurse. And one day he was stunned to observe a patient craning her neck to try and read from the computer trying to view what Jane had typed. When he looked at the screen himself, he was flabbergasted to observe it was a file about a well-known patient of his who was having some rather embarrassing health problems.

He would also find that he had lesser time to spend with his family due to the fact that an inordinate amount of his time was spent looking for information in the patient records and then correcting wrong entries. He would question Jane about it only to be told that she only had so much time to do everything. Sometimes he would find that whole tapes of his recordings had been misplaced and therefore big chunks of information would be missing from Patient records, sometimes entire files would be missing.

He tried to outsource the excess work of transcription to an independent contractor (IC) and soon found that the rates were high and he had to spend a lot of time explaining to the IC what he ‘actually’ wanted. He also was not assured of uninterrupted services during weekends and Holidays. In addition, more time was spent on coordinating the files transcribed by his secretary and those transcribed by the IC.

And when claims were filed for reimbursement, he would find that it was very difficult to bill the patient for the rejected amount of the claim as too much time had passed since the patient’s visit.

One day he got in touch with TransDyne (www.transdyne.com), a full service medical transcription provider. The people at TransDyne heard his office woes and offered to take care of his medical transcription needs. He was not sure but decided to listen to their offer. TransDyne offered him a medical transcription plan with the following benefits:

·        He could retain his current mode of dictation
·        Transcribed files would be returned with a turnaround time of 24 hours. If he required STAT files sooner, they could be returned within 4-12 hours.
·        The files would have a guaranteed accuracy of above 99%
·        All his weekend and holiday transcription work would be covered.
·        He would be provided with the benefits of an online system, which would help him trace his dictations and transcribed documents easily. His dictations would be uploaded automatically and he would have the options of printing finished documents automatically, if required
·        The information would be completely secure
·        The cost per line of transcription would be 10 cents per 65-character line
·        Any specific formats and templates he wanted could be incorporated
·        Medical transcriptionists who were specifically trained in his specialty would transcribe his files
·        TransDyne had the ability to transmit information from the transcribed files into an EMR system if required
·        TransDyne would offer him a free trial period to enjoy their services before deciding to sign with them
·        He could talk to TransDyne’s customers who had benefited from similar services

Dr.Williams checked with a couple of satisfied customers and decided to avail a free trial of TransDyne’s medical transcription services. He was pleasantly surprised at how easy and convenient it was.  In case he or his staff had any queries TransDyne would attend to them immediately. He was thrilled to see that his medical transcription costs had come down by nearly 50%. Information was available on time and was captured accurately. The technology features offered by TransDyne made life easier, by making it easy to locate any information. He was also happy to observe that the entire process was secure. Dr. Williams decided to sign a contract with TransDyne to avail their services

In the next few weeks, Robert informed him that his funds flow had improved and reimbursements were happening faster because they were now able to file claims much sooner than they were doing earlier.

Medical transcription: Understanding cost

Medical transcription is the process of converting audio files of the patient-healthcare professional encounter into text format. These records form a part of the permanent patient record and are used for various purposes, including coding and billing for the purpose of reimbursements. Medical records are a valuable resource to the healthcare professional for providing quality healthcare on a timely basis.

In spite of being an important part of the healthcare process and an accelerant of the receivables cycle, medical transcription is one of the cost factors. To understand the cost of medical transcription it is important to evaluate all options available for availing medical transcription and evaluate the cost components of each method.  A healthcare facility can avail medical transcription services by the following means:

By having an in-house transcription team: To calculate the cost per line of transcription under this method, the healthcare facility needs to include indirect costs like cost of real estate, overheads, utilities, managerial/supervisory costs, cost of training, cost of repairs & maintenance, cost of weekend & holiday transcription, cost of IT along with the direct costs like Salaries and benefits paid to the medical transcriptionists. Factors like cost of overtime and loss of productivity during slack hours also need to be added to arrive at realistic costs.

By having independent contractors working for them: To calculate the costs under this method, apart from the cost per line of transcription, factors like cost of coordination of transcription activities, cost of in-house IT towards medical transcription, server costs for upload & download of files, cost of equipment & software provided to the Independent contractors, cost of security measures, needs to be added to arrive at the actual cost per line of transcription.

By outsourcing to a professional medical transcription service provider: Under this option the cost per line of medical transcription is clearly defined by the service provider. However the medical transcription service provider needs to evaluate on the accuracy, turnaround time, security, technology used and the billing method before arriving at the final costs.

By having a combination of the above methods: Using more than one source to meet medical transcription needs can lead to complexities like different methods of billing, different methods of calculating accuracy, different technology used by different providers, cost of coordination and the additional cost of ensuring security of data. This method could probably create confusion and additional work.

In conclusion, it can safely be said that outsourcing to a professionally managed medical transcription company can help the healthcare facility curb costs and limit it to the cost per line of transcription. However while doing so, care needs to be taken to evaluate the medical transcription service provider on the following:

·        Billing method
·        Security measures taken by the medical transcription service provider
·        Quality measures to obtain accuracy
·        Turnaround time
·        Technology

TransDyne is a professionally managed medical transcription service provider.
